Our Program
Our primary aim is to welcome every person to our home with open arms. We designed our home with the philosophy that we wouldn’t expect anyone to live in a home that we ourselves couldn’t live in. Envisioning yourself being comfortable and happy in a home is just a start to our list of priorities.
We provide 24-hour community-based Customized Living housing and services for adults living with a Serious and Persistent Mental Illness (SPMI) or other disability. We help individuals maintain independence while also being a part of the larger community. Our Customized Living program is overseen by a registered nurse and may include:
Room and board (3 meals & 2 snacks)
24-hour staffing
Advocacy
Appointment management
Medication management
Home health services
Housekeeping
Socialization and activities
Qualifications
18 years of age or older
Diagnosis of a mental illness, brain injury, or other disability
Willing to adhere to care plans created by medical providers
Public funding available:
Services are funded through the Waiver Programs (CADI, BI, EW)
Room and board is funded by GRH (Group Residential Housing), or paid privately (SSI, SSDI, RSDI)

135th House is a gorgeous 5 bedroom home that has an open floor plan. It has been remodeled and boasts a lot of finishes and uogrades. We have 3 bathrooms and an extra living space downstairs for residents. We offer cable and internet to all our residents.
Columbus House
This large 5 bedroom 3 bathroom home hosts an open floor plan, large kitchen with a breakfast bar, formal dining area, first floor laundry room with a finished lower level family room. We offer cable and internet to all our residents.
